Language of the form

The Beijing Municipal Government English portal carries a guide to the health entrance examination. The page carries the date 2022-12-16.

For a form completed overseas, the guide says the input fields should be filled out in Chinese or English. It says the date of issuance should be clear, and the form is valid within six months from the date of issuance.

Certification by a consulate or embassy

The Beijing guide says that if the content of the physical examination report needs to be certified by the Chinese consulate or embassy in a foreign country, the applicant must provide the certification proof or label to health centre staff.

The guide adds that the consulting physician adds corresponding items of physical examination case by case if the certification proof or label is missing, if fields are missing or unclear, or if the original reports are not attached.

Translation of work permit materials

A page on the Beijing Municipal Government English portal says all materials in a language other than Chinese must be accompanied by a Chinese translation and stamped with the employer's official seal, except for passports and other international travel documents.

The page says all original hard copies of the materials must be uploaded electronically in color to the online processing system together with the Chinese translations.

Collecting the documents

The Beijing guide says that after passing the physical examination or the verification, the applicant or another person entrusted collects the documents at the issuance window with the Document Collection Notice within the time period specified by that notice.

The Shanghai Municipal Government English portal carries a guide to the health check. The page is dated August 30, 2024, and its footer says it was updated July 14, 2026. The guide says that three working days after the health check, the health certificate can be picked up in person at Window 2 of the Public Service Center by presenting the original valid ID and the certificate collection receipt.

Where the certificate is used

The State Council's Regulation on the Administration of Entry and Exit of Foreigners says foreigners who apply for a residence permit valid for 1 year or more must submit a health certificate as the rules provide. It says a health certificate is valid for 6 months from the day it is issued.

The Shanghai guide says that for people planning to live, work or study there for a year or more, the health check at the International Travel Healthcare Center is crucial for applying for residence and employment permits.

A Beijing manual for non-credit-based foreign professional talents says that when applying for a Foreigner's Work Permit after entry, the Verification Certificate of Physical Examination Records of Overseas Personnel or the Certificate of Health Examination issued by the Inspection and Quarantine Institutions of China has to be provided.
